I think Herbert is responsible for his death. Even though Mr. White made the first wish Herbert was the one who wanted the 200 pounds . I also think Herbert knew he was going to die when the made the first wish and the money didn't come .And when Herbert said that he shall never see the money.

I think Mr. White was the most responsible for what happened. He didn't heed the warning from Sergeant- Major Morris either. The warning was this "If you keep it, don't blame me for what happens." He also ended up grabbing it off the fire and ended up making a wish that resulted in his sons death.
